- LEVEL 1 - Low Level Care (Minimal Assistance Needed in performing ADLs)
- LEVEL 2 - Moderate Level of Care (Requires Hands-On Assistance in Performing ADLs)
- LEVEL 3 - High Level of Care (Requires Hands-On for one or more ADLs from more than One Caregiver)
